1. Capture the consult
Start the recording on your phone, tablet or desktop. The Scribe listens ambiently while you stay with the patient — no typing, no dictation script.
2. Draft a structured note
The conversation becomes a note in your own template — SOAP or discipline-specific — with subjective, objective, assessment and plan sections already populated.
3. Code and enrich
ICD-10 and CPT codes are suggested from the note. Past conditions, medications and allergies are extracted into the patient profile, and photos of forms or referrals become structured data.
4. Review, sign, lock
You edit anything you want, then approve. Only a clinician-approved note is locked into the record, and the invoice and any letters build from that same note.

What is an AI medical scribe?
An AI medical scribe listens to the consultation, transcribes it and turns the conversation into a structured clinical note. How long does it take to get a note back?
The draft note is available at the end of the consultation, in seconds rather than minutes. You review, edit and lock it before it becomes part of the clinical record, so the note is finished before the patient has left the room.
Does the AI Scribe write SOAP notes?
Yes. SOAP is one of the built-in structures, alongside discipline-specific templates for physiotherapy, occupational therapy, psychology, dentistry, dietetics and general practice. You can adapt templates to the wording your practice already uses.
Can it code the visit for billing?
The Scribe maps the note to ICD-10 diagnosis codes and procedure codes, and hands them to invoicing so the claim is built from the note itself instead of being re-typed. The clinician confirms the codes before anything is submitted.
Is patient data safe with an AI scribe?
Recordings, transcripts and AI outputs stay inside the HeroMed platform under the same controls as the rest of your data — encrypted in transit and at rest, role-based access and full audit trails, built around POPIA, HIPAA and GDPR requirements. AI providers do not train on your patient data.
Do I have to buy a separate scribe subscription?
